Error in n-back or just me?

Sometimes I get a blank gray square with no white square in it. Is it an error and if not, how should it be interpreted?

That should be an error; it is not supposed to happen. It might be some kind of hiccup in the timing.

It must happen quite a lot, huh? Ohh, bother. Time permitting, I'm looking at rebuilding most of the tests in the next couple of weeks.
